We are looking to recruit an enthusiastic and committedNurse into the role of Senior Band 6 Nurse based within the DORNeighbourhood Team. It is envisaged thatthe individual appointed will be primarily based at Droitwich Medical Centre,but will also work across the Neighbourhood Team, including at our satellitebase in Tenbury. There will be necessity to carry out clinical visits topatients within their own homes depending on the needs of the service, makingthis a really exciting and diverse Role.
Our Neighbourhood Team is an integrated team of Registered Nurses,Physiotherapists, Occupational Therapists, Health Care Assistants, AdvancedClinical Practitioners, and Administrators and work together in our localNeighbourhood and alongside our GP partners to support people in their home environment.

The service operates from 7:30am 6:30pm,Monday to Sunday including bank holidays. Some weekend work will be required. The post holder will:

Demonstrate expertclinical knowledge, health promotion and holistic assessment skills and willwork with the patient and their carers to assess, plan, implement and evaluatetheir ongoing care and support needs in their own home or temporary/usual placeof residence. Support theOperational and Clinical Team leaders in the delivery of best nursing practice,be involved in triage, allocation of work and leadership of other staff, manageteam resources and deputise in their absence.
