Job Title: Memory and Wellbeing Guide

Based: With a base in Cynon Linc, Aberdare, CF44 7BD and at satellite bases throughout the region of Cwm Taf Morgannwg

Hours: 18 hours per week

Salary: £12,466.22 per annum

Contract Length: Until end of March 2026

Job Description:

As a named contact for those in assessment process, newly diagnosed with dementia or those with an MCI, the Memory and Wellbeing Guide will ensure that:

  • People are connected to available and appropriate support networks, which may include peer support options.

  • People are able to access practical advice and information, including financial, to support with the adjustments to their lifestyle, or environment, that will help them remain as fit and healthy as possible.

    The Memory and Wellbeing Guide as defined nationally will help people living with dementia coordinate care alongside the statutory duties of Health and Social Care.

    Responsibilities:

    The duties and responsibilities are informed by what matters to people living with dementia in the Cwm Morgannwg region.

  • To establish the Memory and Wellbeing guide role as a point of access for information and advice to people living with dementia or MCI, throughout their journey and confirm the frequency of check in conversations they require.

  • On first contact to facilitate a ‘what matters to me’ conversation to consider the person and their carer or family’s need for ongoing support and facilitate through the provision of information and advice and signpost as necessary.

  • To be responsible for recording the ‘what matters to me’ conversation and the information and action plan agreed as a result.

  • Within 12 weeks, to connect with the person, their carer and/or families to facilitate a further ‘what matters to me’ conversation.

  • To always use the ‘what matters to me’ conversation when contacted by people living with dementia, to fully understand their situation and what they need and to be responsible for recording the conversation and the information given and action plan as agreed as a result.

  • To maintain strong relationships with all appropriate local health and social care partners, including adult social care, primary care, OPMH services, third sector and community-based groups and services to facilitate connections and referrals for people living with a dementia, where appropriate to do so.

  • Facilitate a responsive point of access response for people living with dementia.

  • To maintain an effective understanding of dementia relevant training available for people living with a dementia and support access as appropriate. Version – October 2024

  • To maintain an effective understanding of the rights and entitlements of people living with a dementia to provide the right information, advice and assistance – including signposting to others.

  • Maintain accurate records and timely responses.

    All staff/volunteers working for Age Connects Morgannwg are expected to respond appropriately to any concerns that they may have regarding the abuse/inappropriate treatment of vulnerable adults.

    The protection of vulnerable adults is a core responsibility at all times.

    The above is a broad definition of the job responsibilities. It does not take into account every aspect of the job which the jobholder may be required to perform. Flexibility is essential since the jobholder’s working hours will be determined by the requirements of the business
