Marie Curie is the UK’s leading palliative and end-of-life care charity. We are the largest non-NHS provider of palliative and end-of-life care in the UK, and the only provider across all 4 nations. We specialise in the provision of UK wide community nursing and hospice care, both inpatient and outpatient, whilst providing information and support on all aspects of terminal illness, dying, death, and bereavement. Our leading research pushes the boundaries of what we know about good end-of-life, and our campaigns fight for a world where everyone gets to have the best possible quality of life while living with an illness, they’re likely to die from. 

The care and support we provide is highly valued by the people we care for and their loved ones, but at present we are only reaching around 10% of dying people at the end of life. Right now, one in four people in the UK with a terminal illness, do not get the care or support they deserve at the end of their lives. 

We are recruiting for a Hub Coordinator to join our administration and support team in our Tiverton Office. 

In this role you will act as a first point of contact for all Marie Curie Services across the South West from the local co-ordination Hub. You will lead and support the referral process, co-ordination and booking of care as required, ensuring data and information is accurately inputted into operational and clinical systems.

You will work with Senior Co-ordinators to ensure the smooth running of the co-ordination hub. You will also work alongside and liaise with clinical support & management staff as well as, volunteers, referrers, local CHC teams, patients’ families and carers.

This is a varied role which requires a high level of diplomacy, excellent communication skills and the ability to work accurately and under pressure. 

This role is based in our Tiverton office, 37.5 hours a week on a shift basis either 08:00-16:00, or 10:00-18:00, with approx. one weekend in four. 

In this role you will:

Be responsible for the daily rota which supports the allocation of Marie Curie Nurses to patients. This includes responsibility for monitoring and highlighting the utilisation of each service to the Senior Coordinator, Senior Nurse and Clinical Nurse Manager. Direct liaise and negotiate where needed with Marie Curie Nurses regarding their ability to cover shifts when they are not on the formal rota. Use the system to add referrals into eCommunity and EMIS/SystmOne Input of new patient details to Marie Curie systems ensuring that all key information is accurate and available to the Marie Curie nurses Communicate with families regarding care provision via telephone. Liaising with referrers to check care packages are in place prior to offering patients care. Ensure all systems are up to date with information for booked visits so that Marie Curie Nurses can undertake their work effectively Coordinate last-minute changes advised by the Clinical Nurse Manager, Hub Manager, and Senior Nurse Listen to patients and carers’ concerns, suggestions and experiences, signposting these to those designing and managing services. Provide non- clinical information to patients, relatives and other healthcare workers on an ad-hoc basis
