We have an exciting opportunity for a Registered nurse or Paramedic to join The Joint Emergency Team [ JET] , to support with urgent community hour response for patients in their own homes. Advanced Clinical skills and community experience working autonomously are essential.

You will be required to work over a day working week, including weekends and Bank Holidays. Hours- hrs and - hrs. We are looking for someone who is hardworking, motivated with excellent communication skills. You will be able to demonstrate knowledge and skills in holistic/clinical assessment .

Main duties of the job

Supporting patients in their own homes working with a multidisciplinary team with our system partners in the Urgent Community Response [ UCR] , preventing ED attendances and unnecessary hospital admissions.

The role is to work collaboratively with other team members offering holistic, high level Clinical assessments and reviews for patients who may be in crisis. To act as patients advocate facilitating choice and patient empowerment. To provide evidence based clinical/therapeutic interventions based on best practice. To work with all health professionals to provide a seamless, integrated service to our service users. To triage referrals on a rotational basis for the Urgent Community Response Team and the Urgent Emergency Care Hub [ UEC Hub].
